Parental Permission Form/Acknowledgement and Assumption of Risk
THIS DOCUMENT MUST BE SIGNED BY THE PARENT OR LEGAL GUARDIAN OF PERSONS UNDER THE AGE OF 18 PARTICIPATING IN THE TRIPLE CROWN OF FOOTBALL.
Lutheran High School – Legends of Sports, LLC – Jenkins Elite and Amata Sports strives to provide a safe environment for its Youth Activity Event (Triple Crown of Football), Participants (the “Participants”). However, it is important for the Participant and his or her parents or legal guardians to understand that even with safety measures, participation in the Triple Crown of Football Event (the “Event”) involves unavoidable exposure to, and inherent risk of, injury. Therefore, the individual named below as the Participant, and his or her parent or legal guardian hereby acknowledge that he or she authorizes the Participant to participate in the Program, and further acknowledges his or her full understanding and appreciation that there are risks of injury associated with participation in the Program. These risks include, but are not limited to, injuries sustained from falling, reasonable sport-appropriate contact, overexertion, allergic reactions to any food or beverage served, allergic reaction to sunscreen applied to the Participant’s skin during the Program, etc. These risks occur in activities including, but not limited to, running, jumping, push-ups, jumping rope, and other indoor and outdoor sport activities and travel to and from Event sponsored activities.
The person signing this document hereby represents that he or she has advised the Legends of Sports of any facts known to him or her which would make the Participant more susceptible to injury or risk of injury as a result of participating in the Event than would be the average person of the same age. Any parent or legal guardian signing further represents that he or she has thoroughly explained to the minor Participant the risks associated with participating in the Event using language appropriate to the age and intellectual capacity of the Participant
By signing this form, the Participant, parent or legal guardian, on behalf of themselves, their heirs, assigns, legal and personal representative(s), agree to assume all risks and responsibilities surrounding the Participant’s participation in the Event and further to release Legends of Sports, LLC, it’s staff, volunteers, coaches, practice facilities, and all departments and divisions thereof from any claims, demands, actions, causes of action, lawsuits, expenses, or losses (including court costs and all reasonable attorney fees) he or she may have on account of personal injury (including death) arising out of or attributable to Participant’s participation in the Program, whether such personal injury or death is caused by the negligence of Legends of Sports, LLC, its trustees, staff, volunteers employees, or agents, or otherwise. The person signing this document also gives permission to Legends of Sports, LLC, to seek reasonable medical treatment on the Participant’s behalf in the absence of the parent/legal guardian.

This waiver shall be effective for all Programs participated in by the undersigned youth for one year after the date executed.

IMAGE AND LIKENESS PERMISSION: I further consent to the use of the Participant’s name, image, and likeness depicting his/her participation in the Event (in any literary, audio, visual, photographic, film, video, or other form) by the Lutheran High School – Legends of Sports, LLC – Jenkins Elite and Amata Sports for promotional material only.
